Finding a Hospice Program

A doctor may recommend hospice services and refer a patient to a hospice program. If the doctor doesn’t recommend hospice, a patient or family member may request hospice services or may contact a hospice program directly to inquire about services.

Locate a Hospice provides information about hospice programs serving Minnesota. Another source of information is the Minnesota Department of Health list of state licensed and Medicare certified programs. This is a list of all hospice programs licensed in Minnesota.
