- Aging with Dignity – National non-profit organization that promotes human dignity as America ages, with emphasis on improved care for those near the end of life. Distributes Five Wishes, an easy to use advance directive that over 1 million American families have used to discuss how they want to be treated during times of serious illness.
- Americans for Better Care of the Dying aims to imporve end-of-life care by learning which social and political changes will lead to enduring, efficient, and effective programs.
- The Commission on End of Life Care Final Report (01/2002) - The Commission on End of Life Care Final Report was produced by the Minnesota Partnership to Improve End of Life Care in collaboration with the Minnesota Department of Health. Major funding provided by the Robert Wood Johnson Foundation Community-State Partnerships grant.
- Family Caregiving at Life’s End is a booklet that describes the signs and symptoms of the dying process, categorized by stages from greater than three months life expectancy to the actual time of death, and offers suggestions for the caregiver at each stage. The booklet was created in partnership by the Minnesota Board on Aging and HealthPartners–Hospice of the Lakes.
- Growth House – Search engine offers access to the net’s most comprehensive collection of reviewed resources for end-of-life care. Inter-Institutional Collaborating Network on End-of-Life Care (IICN) links health care organizations through a shared network.
- Caring Connection is a national consumer and community engagement initiative to improve care at the end of life, providing free resources and information.
Hospice & Palliative Care Organizations
- National Hospice and Palliative Care Organization (NHPCO) – The largest nonprofit membership organization representing US hospice and palliative care programs and professionals. Develops public and professional educational programs and materials; convenes meetings and symposia on emerging issues; provides technical informational resources to members; conducts research; monitors congressional and regulatory activities; and works closely with other organizations interested in end-of-life care.
- American Academy of Hospice and Palliative Medicine – is an international organization of physicians dedicated to the advancement of hospice and palliative medicine.
- Hospice and Palliative Nurses Association – is a national professional nursing organization dedicated to promoting excellence in hospice & palliative nursing care.
- Hospice Foundation of America – provides end-of-life care resources for professionals, patients and families.
- Minnesota Department of Health list of state licensed and Medicare certified programs
- The Center to Advance Palliative Care (CAPC) provides health care professionals with the tools, training and technical assistance necessary to start and sustain succesul palliative care programs.
