Hospice services are provided by a team of specially-trained professionals and volunteers. The hospice team includes a doctor, nurse, social worker and spiritual caregiver. Home health aides, therapists and volunteers may also be part of the team. The patient’s personal physician works with the hospice team in providing care. The patient and family are the core of the hospice team and are at the center of all decision making.
