Director of Home Health & Hospice

Mayo Clinic Health System La Crosse, WI is seeking a Director of Home Health/Hospice, responsible for planning, directing and coordinating operational and personnel activities to ensure the provision of quality services to meet the needs of those served by the Home Care/Hospice Department. The Director provides leadership based on the Franciscan Healthcare mission, vision, and values. The role includes ongoing collaboration and communication utilizing shared goal setting and decision making with physicians, associate providers, department staff and multidisciplinary team members. Qualifications:
• RN License or eligible in state(s) of practice.
• MSN or related master’s preferred.

Hospice Family Support Coordinator

Our interdisciplinary hospice team works with the patient and their family to create a personalized plan of care for both patients and families. The plan of care will address physical, emotional and spiritual needs with a focus on comfort.

We currently have a full time opportunity for a professional with experience in grief and loss support services to coordinate activities of hospice staff and volunteers in the provision of support to survivors, patient, family or other caregivers including children, adolescents, adults and geriatrics who are deemed at risk for problems in the bereavement period. This individual will also provide support to hospice staff as appropriate.

Qualifications:

  • Bachelor’s degree required; Master’s in Human Services or related field preferred.

BEREAVEMENT COORDINATOR

As a member of the interdisciplinary team, this person is responsible for assessing the bereavement needs of Hospice and Palliative Care families and provide follow up bereavement support. Make referrals to specially trained volunteers or other community resources for additional bereavement services as needed for complicated grief. Implements and abides by Customer Service Standards. Supports and implements patient safety and other safety practices as appropriate. Supports and demonstrates Family Centered Care principles with patients and their families and with co-workers.

EDUCATION
Advanced degree in counseling, psychology, social work or ministry required.

EXPERIENCE
Experience in counseling and dealing with grief, loss, and end-of-life issues required. Demonstrates clinical experience in the areas of life-threatening and chronic illness, grief, and loss required. Knowledge and ability to work with interdisciplinary team. Strong organizational and group facilitation skills and ability to prioritize required.

SPECIAL REQUIREMENTS
Transportation available for off campus activities required. A valid Minnesota driver’s license, a driving record in good standing, and the minimum amount of insurance liability coverage required.

Hospice Home Health Assistant On-Call

We are currently seeking an on-call Hospice Home Health Assistant to join HealthPartners Hospice and Palliative Care group. This position is responsible for assisting terminally ill patients in maintaining their personal care requirements, maximizing their Activities of Daily Living (ADLs), and achieving the highest level of comfort and quality of life in their home consistent with their health status.

Additional responsibilities:

1. Assist patients with personal and self-care skills in the home environment.

A. Assist with common personal hygiene and self-care requirements.
1. Bathing with or without assistance
2. Foot soaks
3. Shampoo
4. Combing/brushing, makeup, shaving, and fingernails as required.

B. Perform personal hygiene tasks clients are unable to perform without assistance.
1. Perform total bath requiring turning and repositioning.
2. Perform perineal care.
3. Perform skin care measures, e.g. application of topical ointments, as directed.
4. Provide for oral/teeth care.

C. Perform or assist the client in performing activities related to mobility – passive range of motion, turning/repositioning, transfers, ambulation – with the aid of another person or assistive equipment or devices as needed.

D. Perform routine observations and treatments as assigned by nurse and report results:
1. Take vital signs: temperature, pulse, respirations, and blood pressure.
2. Do simple, non-sterile dressing changes.
3. Maintain routine bowel and bladder programs.
4. Do daily Foley catheter and condom care.
5. Do routine ostomy cares, e.g., changes ostomy collection bags.
6. Perform other procedures after direct instruction at the discretion of the nurse.

E. Perform or assist the patient in performing the normal activities of daily living such as eating and dressing.

F. Assist the patient in taking self-administered oral medications.

G. Use encounters with patients as an opportunity for psychosocial support to enhance morale, observe cognitive functions and observe for and maintain environmental safety.

H. Perform light housekeeping duties to maintain the patient’s level of function: prepare light meals, change bed linens, clean and straighten the patient’s immediate personal environment.

2. Report significant observations and changes in the patient’s condition to the nurse in a timely way.
3. Participate in care conferences during which patient care is planned or reviewed.
4. Document all patient-care activities as assigned by the nurse in a timely way according to departmental policies and procedures on the appropriate forms.
5. Maintain required statistical data.
6. Implement infection control and safety measures in patient care following departmental policies and procedures.
7. Demonstrate concern for quality assurance issues related to position responsibilities.
8. Uphold the rights of individual hospice patients and demonstrate a commitment to excellence of service.
9. Participate in education programs to maintain certification and enhance skill development.

This position requires:

• Certified as a home health aide or nursing assistant with a 75-hour program.
• Six months of successful previous experience as a home health aide or one year as a certified nursing assistant.
• Able to complete competency testing prior to employment.
• Current CPR certification upon hire or within six months of employment
• Current MN Driver’s License and daily access to reliable, insured vehicle.
• Computer literacy.
• Able and willing to drive a car in weather conditions typical of the upper Midwest
• Must be able to speak, read, write and understand English.
