What is an Advance Directive?

A Health Care Directive is a document in which you state your wishes and preferences about health care treatment decisions—who should make them for you and how you hope those decisions will be made. A Health Care Directive is a plan—it is intended to guide treatment decisions in the event that you can no longer make those decisions for yourself.

There are a many different health care directive forms available that meet the legal requirements in Minnesota. It is not necessary to have an attorney provide or fill out the form. In fact, it is not necessary to use a pre-printed form at all. Any written statement that meets the requirements stated above can serve as a legal health care directive.
