2012 Hospice Volunteer Service Award

MNHPC is now accepting nominations for the 2012 Hospice Volunteer Service Award! Each year, the Minnesota Network of Hospice & Palliative Care (MNHPC) receives nominations for the Volunteer Service Award. This award honors an individual who has made a difference in the lives of terminally ill individuals and their families throughout Minnesota. The winner is selected by the MNHPC Executive Committee and is publicly recognized during lunch on Tuesday, April 17, 2012 at the annual conference. The award recipient receives free registration for the conference (one or both days) and the opportunity to invite 2 guests to the Tuesday lunch.

Nominations are due Friday, February 24, 2012. Download the 2012 Hospice Volunteer Service Award Nomination Form. Volunteers are evaluated based on the following criteria:

  1. Is currently a volunteer with hospice and has been for at least three years. The  volunteer must work directly with patients residing in Minnesota and may serve in other capacities as well.
  2. Exemplifies the hospice philosophy and serves as an effective ambassador on behalf of hospice.
  3. Has demonstrated exceptional service to hospice.
  4. Demonstrates to those with whom he/she works a thorough understanding of the role of the hospice volunteer.

Remember, the most effective way to promote the nominee is by clearly stating what sets this person apart as a hospice volunteer. Provide examples and/or stories to illustrate the person’s uniqueness. Selection will take place on Friday, March 16 and the winner will be notified shortly thereafter.
