About Opening Doors

Our Goal is to increase access to end-of-life care through community and provider education. This includes communicating best practices, distributing translated materials and connecting hospice care providers to diverse communities seeking hospice and palliative care.

Our Objectives are to:

  • Increase patient, family and community knowledge and understanding about hospice resources and care among multicultural populations in Minnesota;
  • Increase the capacity of Minnesota’s hospice organizations to serve end-of-life needs among minority and multicultural communities; and
  • Develop and disseminate culturally competent resources for patients, families and hospice providers.

Our Outcomes are to:

  • Share and expand culturally appropriated translated hospice and end-of-life education materials with patients and families from diverse cultures;
  • Promote best hospice practices in serving patients from diverse communities;
  • Provide culturally appropriate training, education and resources to hospice and end-of-life care providers throughout Minnesota;
  • Engage Minnesota’s multicultural populations in ongoing conversations, training and education with hospice providers on sensitive end-of-life care for diverse communities; and
  • Increase access to end-of-life care and use of hospice services by Minnesota’s multicultural populations.

You can participate by helping to share cultural competency hospice education and resources. Contact Minnesota Network of Hospice & Palliative Care at 651.659.0423/800.214.9597.
