MNHPC's Hospice Care Concierge will assist with resources, conversations, and questions about hospice and/or palliative care. Concierge sessions via phone or zoom are available by appointment. MNHPC representatives can assist you with finding care, questions about current care, offering emotional support, directing you to resources, guidance on financial questions, and other end-of-life-related help.
